Here are instructions to insert a Wufoo (wufoo.com) form onto a workspace page:

1. Sign-up for a free account and create a form at http://wufoo.com;
2. After creating a form, return to the "Form Manager" screen;
3. Click on the "Code" button for the form that you wish to insert to your workspace page;
4. On the "Code Manager" screen, click on the "Embed Form Code" option on the left;
5. Select the entire code that appears and copy it (CTRL+C / Command+C);
6. In the "Edit" mode of the workspace page where you wish to insert the code, click on the "Insert Plugin" feature;
7. Select the "HTML/Javascript" plug-in from the "PBworks Magic" category;
8. Paste the copied Wufoo code into the plug-in (CTRL+V / Command+V);
9. Click "Preview" then click "OK";
10. Finally, click the "Save" button at the bottom of the page to finish adding the form.
